To create a form that you can use in transactions going forward, you must do so from the Library screen
In order to create a form that you can use in Pathways going forward, you must do so from the Library screen. Here’s how you create one:
Navigate to the Library Screen, and make sure that you are in the form section of the page (the form tab should be highlighted in blue (shown below).
Click the ‘Create Form’ button near the top right of the page.
Build a new custom form in the form builder by adding as many new fields as you require, with all the necessary field types.
Once you have finished the form, click the ‘Publish Form’ button at the bottom right of the screen. This will save your form to the Library for future access in Pathways.
